The procedure for application to the Day Treatment program include:
School system identifies youth in need of DT service based on criteria
School system receives permission from parent/guardian, via signed authorization to
release information form, to pursue assessment and treatment services from ATS
School
system presents the youth's case to Student Support to propose approval for DT services
DT receives completed application packet, to include teacher and CDT staff observations
and a current physical exam (within the previous 3 years)
Diagnostic assessment is made
indicating the need for Day Treatment. The assessment is developed from a review of the
application, family assessment, eligibility criteria, hospital discharge summaries, educational
IEP’s, social history
and/or psychological evaluations
DT staff meets with parent/guardian to complete additional
paperwork, discuss program goals and expectations and address questions or concerns
